I confirm that 'the player' has read and agrees to comply with the Respect Young Players Code of Conduct.

I can confirm that all parents/carers in relation to 'the player' have read and agree to comply with the Spectators and Parents/carers code of conduct.

I understand it is my responsibility to keep my linked FAN up to date with emergency contact & medical info and to advise the manager of any specific changes or concerns.

I understand that the club will follow all Covid 19 guidelines as advised by Government and the FA and agree to abide by these.

If required, I agree to stay on site with my child and understand that in the first instance I may be required to give first aid.

I give permission for my child to have their photograph taken.

I give permission for my child's full name and Image(s) to be used on the club website & in the media.

I give permission only for my child's image(s) to be used on the club website & in the media.

I understand that these images may be used for club publicity purposes.

I confirm that I have answered all the above to the best of my knowledge.

I have read and understand that any match or training clothing that is supplied by the club will need to be returned if the player leaves or if the club require it back at anytime, (lost or replacement items can be ordered through us at cost.)

I agree to return all items when required.
